Coronary perforation during balloon anchoring after rotational atherectomy of chronic total occlusion of coronary vessel

چکیده
An 87-year-old Asian male experienced progressive shortness of breath for two days. He presented symptoms and signs of decompens-ated heart failure and was diagnosed as non ST-segment elevation myocardial infarction, killip III. Coronary intervention was performed for chronic total occlusion of left anterior descend-ing artery. Unfortunately, coronary perforation happened during balloon anchoring after rotational atherectomy. Rotational atherectomy may be not related to perforation, but causes vessel wall more fragile after disruption of the arc of calcification. Even if balloon anchoring was performed at low pressure, we need keep in mind that it still could result in a terrible coronary perforation after debulking techniques. Covered stents for emergency implantation in case of coronary perforation must be an obligatory inventory of catheterization laboratories. It is an effective alternative that can be used to seal a major coronary perforation. (This page in not part of the published article.) Rotational atherectomy via transradial access in the percutaneous treatment of chronic coronary total occlusion with rare complication of coronary guidewire transection
Introduction C total coronary occlusion remains one of the limitations of percutaneous transluminal coronary angioplasty, and few therapeutic devices are specifically designed to address this problem. Despite advances in device technology, the management of resistant, calcific lesions remains one of the greatest challenges in successful chronic total coronary occlusion intervention. Established...

Objective: The aim of the study is to investigate the effi cacy, complication rate and predictors of rotational atherectomy (RA) usage after successful guidewire crossing during percutaneous coronary intervention (PCI) of coronary chronic total occlusion (CTO). Methods: A single center experience of 525 consecutive patients from October 2010 to June 2014. A total of 587 CTO lesions were treated...

An 83-year-old man presented with recurrent acute coronary syndrome (ACS) at the left main coronary artery (LMCA) complicated with ostial chronic total occlusion (CTO) in the right coronary artery (RCA) (RCA-CTO). At the first LMCA-ACS approximately 1 year earlier, he had undergone LMCA-crossover stenting with a biolimus-eluting stent in the presence of RCA-CTO. At the second LMCA-ACS, we angio...
متن کاملOptical coherence tomography findings after longitudinal ablation for an underexpanded stent in a heavily calcified lesion: a case report
BACKGROUND Heavy coronary artery calcification is responsible for stent underexpansion, which is associated with increased in-stent restenosis. Here we report a case in which optical coherence tomography (OCT) demonstrated that the metal component of an underexpanded stent previously implanted in a heavy calcified lesion had been completely removed after ablation with rotational atherectomy. ...
متن کاملDouble anchoring balloon technique for recanalization of coronary chronic total occlusion by retrograde approach.
A retrograde approach through the collateral channels is considered to improve the success rate of percutaneous coronary intervention for coronary chronic total occlusion (CTO). Various kinds of strategies and techniques are required to improve the success rate of this novel approach. We describe a case in which a CTO was successfully recanalized by the retrograde approach, using a new anchorin...
